Registration Form (Preschool-Grade 12)
If you would like a welcome folder with the printed forms and other information, call or stop by any of our school offices. At this time we have openings for all ages/grades. We request your registration form as soon as practical in the spring so we have information for staffing and supplies.
In addition to completing the registration form, take note of the following opportunities.
- Apply for STO (School Tuition Organization) tuition assistance if you might qualify. The 2024 deadline is May 3.
- If your student is eligible for an ESA (Education Savings Account) to cover tuition, make sure to apply April 16-June 30 for fall enrollment. Once a student has an account, the child stays in the program but you do need to submit the application annually so the state funds the account for the fall.
